3, Importer or manufacturer (US) information: including name, address and telephone number

4. Contact information of the holder of the test result file supporting the certificate: name, address, email address and telephone number. (Usually understood as a manufacturer outside the United States)

5. Product production date and address, production date to month, address to city

6. Detection time and address or detection report based on certificate
